  • Photos of carved concrete or hardscape work you've finished [how many: confirm]
  • Your business: name, website or social page, states you serve
  • Years in the trade [minimum: confirm]
  • Insurance [confirm whether we require proof]

Approval criteria are pending (decision M4).
